Rukia is more important by far. There is the love interest to take into consideration. I mean, He doesn't really love Orihime, but she loves him.They are childhood friends, that is where they draw the line. The sheer level of abuse that Ichigo takes to save both of them is nauseating, But still Ichigo has a side to him that he doesn't show to anyone but Rukia. And the fact that they fight together on a regular basis helps. (Orihime and Ichigo really never fight together. She is more of a Field Medic.)

For Ichigo no one is more important then the other.He doesn't think anything more special of Orihime then of Rukia or the other way around.He cares for them exactly the same.
