'Desert Punk' used a live action opening sequence for the first half of the series...

The move 'The End of Evangelion' had some live action dream scenes, with cameos of the original women voice actresses...

The ending sequence of the original 'Macross' series showed someone's hand paging through a book of drawings...

The series 'The Irresponsible Captain Tylor' (TV) had as a backdrop, a live action singer in the opening sequence...

The series 'Macross Frontier' (currently showing in Japan) had the silhouette of a live action singer in some of the many closing sequence...

Furi Kuri/FLCL also used live action shots in the ending I believe. Mostly of a scooter.

Well they also have make a live shooting at Air Gears too :P
